Based on information from the Japanese government regarding the COVID-19 booster shot (3rd vaccine), Urasoe City is currently preparing for booster shots in cooperation with relevant facilities toward the establishment of a system for dispatching vaccination tickets.

As details are solidified, we will provide continuous updates via publications, the city homepage, and social media.

Period of Implementation

December 1, 2021 to September 30, 2022

Vaccination of medical professionals to begin from December 2021
Vaccination of the general public (elderly) to begin from February 2022

Intended Recipients

People 18 years or older at the time of their booster whose 2nd vaccination was 8 or more months ago may receive one booster shot.

It is possible to receive the booster shot on the same day that 8 months have passed since one’s 2nd vaccination.
(If the same day that you got the 2nd vaccine 8 months ago doesn’t exist on the month of your booster shot, you can get the booster shot from the 1st of the next month.)


(Example)
- If you received your 2nd vaccination on June 1st 2021, ⇒ you can receive your booster shot from February 1st 2022.
- If you received your 2nd vaccination on June 30th 2021 (the same day doesn’t exist 8 months later), ⇒ you can receive your booster shot from March 1st 2022.

Definition of 8 months or more (Ministry of Health, Labour and Welfare) (Japanese)

Accelerated Vaccinations (persons eligible for booster shots without waiting 8 months or more)

If the below categories apply to you, you may receive your booster shot without waiting for more than 8 months after your 2nd vaccination.

Those who can receive their booster shot at 6 months or more from their 2nd vaccination

・ Medical professionals
・ Residents and employees at elderly care facilities.

Please confirm with a medical facility or the facility you’re staying at regarding accelerated vaccination and vaccination methods.

* If a vaccination ticket is required for accelerated vaccination, please download the medical examination slip request form from the link below and submit it to the city. Please submit it for each medical institution or facility, regardless of whether it is outside Urasoe City.

Those who can receive their booster shot at 7 months or more from their 2nd vaccination

・ Elderly (over 65 years old) * Those who are 65 years of age or older are those born on or before April 1, 1957.

If you are over 65 years old, we will send you a 3rd vaccination ticket 7 months after the second vaccination.

Cost of Vaccination

Vaccines are free of charge (publicly funded).

Vaccine types used

Regardless of the type of vaccine used for your first or second time, MRNA vaccines (Pfizer or Moderna) will be used.

For the time being, the pharmaceutically approved Pfizer vaccine will be used. Use of the Moderna vaccine for booster shots will be discussed after awaiting the result of its pharmaceutical approval.

How to make a booster shot reservation

Those who are medical professionals may be able to receive it at the medical institution where they work, so please check with your employer.

For everyone else, you can place a reservation by calling the city call center as soon as your vaccination ticket arrives.

Vaccination system

As with the 1st and 2nd vaccinations, Urasoe will be setting up a venue for group vaccinations and designated Urasoe medical facilities will be carrying out individual vaccinations.

About the 3rd vaccination ticket

A vaccination ticket is required for the booster shot

A third vaccination ticket will be issued to those 18 or older who have had their 2nd vaccination.

Your ticket will be sent to you at around 7 and a half months after your 2nd vaccination date.

Vaccination tickets will be sent to the address recorded on the certificate of residency of those who have been vaccinated twice and possess a certificate of residence in Urasoe City.

If you have moved into Urasoe City after your 2nd vaccination, you may need to apply for a vaccination ticket to be sent to you.

 

 

Tickets sent to medical professionals

Tickets for medical professionals who received their 2nd vaccination in March or April 2021 were sent in early December.

Tickets for medical professionals who received their 2nd vaccination in May 2021 were sent in late December.

Tickets sent to the general public and the elderly

The timing of the arrival of your booster shot ticket will vary depending on the date of your 2nd vaccination.

They will start being sent from mid-January 2022 based on the 2nd vaccination date.

They will arrive after 7 and a half months from your 2nd vaccination date.

Those who moved into Urasoe City after their 2nd vaccination

If you moved into Urasoe City after receiving your 2nd vaccination, you do not need to apply for a booster shot ticket if it can be confirmed that you’ve received your 2nd vaccination. (Meaning someone whose records at their previous municipality can confirm that they've received the 2nd vaccination.)

The vaccination ticket will be automatically sent to those whose 2nd vaccination was at least 7 and a half months ago.

At present, booster shots are intended for those 18 years or older.

Those who need to apply for the booster shot ticket

Those who wish to receive their booster shots but cannot confirm their past vaccination history or have lost their tickets will need to apply for their booster shot tickets.

Please apply electronically using the below QR code, by post, or at the customer service window.

・ Those who have moved in and out of multiple municipalities after having received their 2nd vaccination.
・ Those who received their 2nd vaccination overseas, or whose number of vaccinations overseas and in Japan equal two.
・ Those who received their second vaccination in Japan from the U.S. military.

Fill out the vaccination ticket application form, and bring personal identifying documentation (driver's license, insurance card, etc.) as well as your 1st and 2nd vaccination certificates to the Urasoe City Health Consultation Center.

Out-of-address vaccination report

For those who will need to receive their vaccination in a city outside of their address, submission (or resubmission) of an out-of-address vaccination report during the period of their booster shots will be necessary. If your certificate of residence is outside of Urasoe City, but you wish to be vaccinated in Urasoe City, you need to submit an out-of-address vaccination report to Urasoe City.
